Insurance Plans
We accept traditional Medicare Part B either alone, or with with their relative secondary gap policy.
We are out of network with all commercial plans.
We will provide you with a superbill upon request, for healthcare services received, with relevant codes at your visit, that may (or may not) be covered by health insurance plans.
Please note: weight loss visits, hormone screening/monitoring and some, (not all) hormone replacement therapy may not be covered by your insurance plan.

Current cash pay prices, due at the time of service
10 minute phone consult visit to see if what we offer is the right fit for you - No Charge
New patient visit $350 - $500 (depending on length of visit/complexity of the visit)
Follow up visits $110 - $350* ( based on the length of visit and complexity)
*Typical follow up visit $175.00 for 20-25 minute visit involving hormone prescriptions
Requested letters, forms, online forms and prior authorizations, $25 to $100 (as of 7/5/2025)
Labwork, some medications and basic labwork tests may be covered under your insurance. Any medication that is compounded is not covered. Any specialty testing will not covered by insurance and we can discuss what your out of pocket costs may be for such testing if/as appropriate for your personal situation.
